The guardien, the guard at the Hopital de Doropo gate, keeps peace and orderliness by allowing only those who are ill or who have a note from another medical institution into the hospital to be seen by the visiting American doctors and nurses.  Availability of trained medical personnel in rural areas of the West African country of Cote d'Ivoire leave a lot to be desired.  This small bush hospital is overseen by an Ivorian-trained nurse, similar to a nurse practitioner in the US.
