Basically this is what I’m looking for answers to.

I want to confirm that I have the appropriate forms. Are the following 3 forms the only ones I need to fill out at this point?

1- ISAF Media Registration Form
2- Media Embed Form
3- ISAF Media Ground Rules and Hold Harmless Agreements

On the ISAF Media Registration Form it asks for the dates of my visit, since they have yet to be determined/approved by ISAF, should that section be left blank? Or should I provide my desired dates? On the ISAF Media Ground Rules and Hold Harmless Agreements, it has a section for dates and location of embed, should that also be left blank at this point?

Will any other documents or items be required with the application? I know, at least of a few years back, a letter from my editor, etc. was required, I don’t see that currently listed.

Sounds like you have the ISAF forms you need for them. Yes, you still need a letter of assignment, a copy of your Afghan visa and U.S. passport. Desired dates, location, unit, etc. works fine.

Would you suggest emailing the paperwork to IJC.EMBEDS@afghan.swa.army.mil even though they have not replied to me? Is there another option?
