581 Hello, I am working on a project that involves photographing and interviewing 8th grade students at a school here in CT. I have obtained permission from the school to photograph inside the school, but now I need to send out a release form to the parents to obtain permission to A) Photograph their children in school. B) to use the child’s first name in the captions of the photos. I also need to include in the release form a section asking [a select number of] the parents if any of them would allow me to document their child’s activities both in and outside of school and their home for a period of 36 hours. Does anyone know of any templates that are available or does anyone have a copy of such a release form that I could modify? Any help would be greatly appreciated.
